I have attached provided count slips to the IA section. stated that she started her morning shift by printing off the roster, and that she made her initial rounds on the unit. stated that rounds are made every 30 minutes. The opening of the door is called "popping the door" within MCC institutional jargon. A "watch-call" is every 30 minutes, with the presence of a Lieutenant, and all door openings are coordinated through "Control". Whenever a "watch-call" is complete, the CO calls "Control" to provide the counted number. "Control" then verifies that the number is correct and documented. "Control's" duties include, monitoring the activity on the ranges, answering calls from COs, replying on the radio, and opening doors. stated that "no one is really moving anywhere within the institution". A count sheet is called a "El", and it is printed off from the internal MCC system called "Sentry". Control validates all responded numbers from the head counts, and marks an "x" on the El sheet to confirm the count. This happens for every check of every unit. El's are supplemented with count slips that properly filled out and stapled to the El time-sheet. Once all head count numbers are verified to be correct, everything is documented, recorded, and then considered to be a good count. began her shift on August 10, 2019 at 12, midnight, to 8 A.M.. stated that the Lieutenant took care of the 12 o'clock count that day. recalled that CO THOMAS and CO NOWELL worked in the SHU on the day of the incident. stated that NOWELL was fairly new. stated that she does not pay specific attention to just one individual screen during her shift since so much is going on. stated that extension "6468" is the number that is called for reporting the count. If a Lieutenant is on the unit for the count, that is when it is considered a "watch-call". Andrea, Correctional Systems Officer SUBJECT: Epstein, Jeffrey Edward # 76318-054 Body Alarm/ Medical Emergency in Special Housing Unit (SHU) On August 10, 2019 at 6:33am while on duty as the morning watch control center officer, Special Housing Unit (SHU) Officer; radioed for medical assistance in SITU and activated the body alarm. I notified staff that a body alarm and medical emergency was called in SHU. Staff responded, than I received a call from staff in SHU telling me to call 911 for an ambulance. I notified the New York Police Department via the institutional NYPD telephone. I then informed NYPD that we need and ambulance and medical assistance for one of our inmates. Staff then continued to respond getting a stretcher and a defibrillator, escorting the inmate down to medical. The New York City Fire Department and Emergency Medical Services (EMS) arrived at 6:43am to the institution and were escorted to the 2"d floor medical area. EMS departed the institution with inmate Epstein and BOP escort via ambulance and chase car to local hospital at 7:10am. CSO / S. Andrea EFTA00132350

In attendance at the interview were A) Office of the Inspector General (OIG) SA , Assistant United States Attorney (AUSA) , and CONTRERAS's attorneys, Andy Patel, Jill Shellow, and Donald Yanella. CONTRERAS was advised of his rights and also that it is a separate crime to lie to a federal agent. CONTRERAS acknowledged what he was advised of and was willing to proceed with the interview. After being advised of the identity of the interviewing investigators and the nature of the interview, CONTRERAS provided the following information: CONTRERAS has been at MCC in the Special Housing Unit (SHU) L Tier since August 2, 2019. His cell is the first cell on the left and he is able to see all the cells on the right side in L Tier. CONTRERAS can also see G and H Tier along with the middle portion of the multi-purpose room in the SHU from his cell. CONTRERAS usually goes to sleep around 11pm or 12am. If he gets to sleep during the day, he will then go to bed around 3am. CONTRERAS stated that he is always up for breakfast which is around 6:10am. CONTRERAS will sometimes wake up when the count is conducted because he can hear the keys. On the morning that JEFFREY EPSTEIN was found unresponsive in his cell, CONTRERAS said that he saw a Guard, who he described as a shorter side heightwise, and had close cut hair, knock on door. The Guard looked into the cell and then entered the Guard opened the door, CONTRERAS could only see EPSTEIN's black male, on the EPSTEIN's cell cell. Both the Guard and EPSTEIN fell back to the floor, with EPSTEIN falling to the side of the Guard. CONTRERAS stated that once the two fell, he could then see EPSTEIN. The Guard then started giving EPSTEIN chest compressions and mouth-to-mouth. At this point the Guard said "Fuck!" and asked the female Guard, who was standing near the L Tier gate, if she had pressed the button. The female Guard said "Yeah, a long time ago." A Lieutenant then came in and also a male nurse who tried to revive EPSTEIN. However, EPSTEIN was not responsive because, according to CONTRERAS, EPSTEIN was already dead. CONTRERAS said that EPSTEIN looked dead when he fell with the Guard after the Guard tried to pick him up. CONTRERAS said that personnel were having trouble getting EPSTEIN onto the stretcher because EPSTEIN was basically dead weight. When they did get him on the stretcher, EPSTEIN still had on headphones. CONTRERAS stated that EPSTEIN did not have any marks around his neck and he didn't see a rope around EPSTEIN's neck. At this point, CONTRERAS woke up his cellmate who then took a look at what was happening. CONTRERAS's cellmate made a comment that EPSTEIN didn't hang himself because he didn't have the typical injuries. CONTRERAS, at some point, saw a male come out of EPSTEIN's cell with a sheet that had a loop and knot. After this, MCC personnel, to include Lieutenant started to cover up the windows to the cells on L Tier. They also brought dividers to block views from his cell and the camera. The day before EPSTEIN died, EPSTEIN and his cellmate left at the same time that morning. EPSTEIN was with his lawyers all day, as usual. However, when he was brought back to the SHU that night, the guards took him up to J Tier before they brought him back to his cell. That night, a count was done a little before 12am, but none were conducted after 12am. CONTRERAS didn't hear anyone say that counts weren't going to be done that night. After being advised of the identity of the interviewing investigators and the nature of the interview, MEDRANO provided the following information: MEDRANO was in the Special Housing Unit (SHU) at the METROPOLITAN CORRECTIONAL CENTER (MCC), , New York, New York, from July 16, 2019 to July 26, 2019. MEDRANO's cellmate was HACINTA (phonetic) GARCIA for this entire time period. MEDRANO recalled that there was a correctional officer (Officer) who was referred to as III that worked the morning shift at MCC, and would remember faces of other Officers, not necessarily names. MEDRANO stated that Monday, Wednesday, and Friday were shower days for inmates in the SHU. These days were the only days that the Officers would pay attention to inmates. Any other time, inmates would have to yell to get something from the Officers. The Officers would do counts of the inmates when they wanted to, not regular counts like MEDRANO knew from previous experiences of being in jail. According to MEDRANO, there were 96 inmates in the SHU and 48 cells. Before the incident happened, MEDRANO didn't remember the overnight checks of inmates actually being conducted by Officers in the SHU. MEDRANO referred to the "incident" as when a "guy" (guy), who's name MEDRANO could not recall but knew him from being in the news, tried to hang himself. After the incident, MEDRANO recalled the checks being done at 12am, 3am, and 5am. He remembered the checks because he would be awaken by the Officers doing the checks. For example, the Officers would say that he could buy his freedom. MEDRANO recalled that the guy would always be with his lawyers and this would hold up other inmates from seeing their attorneys. MEDRANO also recalled that the guy's cellmate used to be a police officer. The incident happened around 1:20am-1:30am. There had been no inmate check at midnight. MEDRANO was reading a book when he heard a commotion. The guy's cellmate was screaming and banging on the cell door to get the SHU Officers' attention. MEDRANO could only see the guy's cell door and could not see in the cell. It took Officer (phonetic) about five minutes to get to the guy's cell but then Officer turned back around to go get the key to the cell. Officer came back with a Lieutenant and two other Officers. MEDRANO described the Lieutenant as middle-aged, white complexion and one of the Officers as black, 6'3", and 260-270 lbs. The Officers opened the cell door and paid attention to the guy's bunkie first. They handcuffed the bunkie and put him inside the shower area. Then MEDRANO heard a big thump like something had hit the floor. He could see the guy's hands stretched out and then saw the Officers pull the guy out of the cell facedown. The guy, only wearing underwear, looked dead due to his skin having a blue tone. The Officers did not administer CPR, they were just looking at the guy. There was no doctor around. The Officers put something that resembled a stretcher on the floor and dropped the guy, who was already cuffed, hard onto the stretcher facedown. At that point, all the inmates started screaming and hitting doors. The Officers then flipped the guy over, dropping him again hard on the stretcher that was on the floor. The Officers and Lieutenant then took the guy away. A while later, around 2:10am-2:15am, the Officers and Lieutenant returned and took the cellmate out of the cell again. They then started to clean the cell. MEDRANO stated that the Lieutenant had a video camera with her, but had it turned in toward her leg to where the view of the camera would not be that of the cell. MEDRANO stated that he saw two orange socks and an orange towel that were tied together taken out of the cell. Once the cell had been cleaned out, the Lieutenant turned the video camera to where the view of the cell would be recorded. MEDRANO was shown a picture of Officer NOEL. MEDRANO said that she looked a little familiar but was not there the night of the incident. MEDRANO was then shown a picture of Officer THOMAS.
